MyMedicare Registration Form

WA has bulk billed phone consults available for patients registered with MyMedicare. 

Patients must have a Medicare or Department of Veterans’ Affairs (DVA) Veteran card to register for MyMedicare. 

Registration is free and voluntary.

By registering as a patient for MyMedicare, additional funding becomes available to your healthcare provider to deliver the care you need.



How to register for MyMedicare 


  1. Click on Download and complete the MyMedicare Registration Form.
  2. Return your completed form to admin@teledocsclinic.com.au
  3. You will receive a link to the booking page once your MyMedicare registration is complete.

Why register with MyMedicare?

Benefits

By registering as a patient, additional funding

becomes available to your healthcare provider

to deliver the care you need.


MyMedicare patients may have

access to:


1. A formalised ongoing relationship with your general practice and preferred GP, which has been shown to improve health outcomes.


2. Longer MBS-funded telephone consultations with your usual general practice, from 1 November 2023.


3. Longer bulk billed telehealth

consultations for children under 16 and

Commonwealth concession card holders, at the new triple bulk billing rate, from 1 November 2023.


4. More regular visits from their GP and better

care planning for people living in a residential

aged care home, from August 2024.


5. Connections to more appropriate care

in general practice for people with chronic

conditions who visit hospital frequently,

from mid-2024.
